Give a Soldier a Smile – Official Rules
1) How to Enter. Enter online at 760KFMB.com. Contestants must be active military or
veteran. Each entrant must submit/upload a picture of his/her smile and a picture of the
inside of his/her mouth; in addition, entrants must fill out a questionnaire regarding their
dental history and describe, among other things, why they should be picked to win. One
valid entry per contestant. Contest entrants agree to abide by the terms of these Official
Rules and by the decisions of KFMB Stations, which are final on all matters pertaining to
the contest. No purchase necessary to enter or win.
2) Entry Period. Entries will be accepted beginning Monday, April 29, 2013 through
Thursday, May 16, 2013. All entries must be received by 5:00 pm on Thursday, May 16,
2013.
3) The Prize. Restorative or cosmetic dentistry treatment for an active or veteran member
of the military. Dr. Peter K. Hellwig, D.D.S., M.A.G.D. will provide the dental services
completely free of cost to one prize winner. Value of prize will vary based on nature of
services performed on winner. Services to be provided at mutually agreeable date and
time, no later than May 31, 2014.
4) Selection of Winner. KFMB Stations will choose five finalists from entries received
based on answers submitted on the entry form. On May 22, 2013, between 12pm and
5pm, the five finalists will each receive a free dental exam and free dental x-rays at Dr.
Hellwig’s office in El Cajon. Dr. Hellwig will then select one prize winner from the five
finalists who have been examined at his office. Dr. Hellwig and his wife Kathy will choose
one prize winner at their sole discretion based on their review of submission essays and
dental needs. The winner will be announced online and on-air on KFMB 760 on May 31,
2013. The four remaining finalists will receive two gift cards worth $100 each towards
dental services with Dr. Hellwig.
5) Eligibility Requirements. Contestants must be active military or veterans, 18 years of
age or older, a resident of San Diego County and possess a valid Driver’s License. Proof
of eligibility is required.
6) Eligibility Restrictions. In addition to the above requirements, the following are
ineligible to participate in the Contest: a) Full and part-time employees, agents,
successors, and assignees of KFMB Stations, its advertising agencies and promotional
companies involved in any Promotion, as well as family and household members of
same; b) persons who have been employed by the KFMB Stations within 60 days
preceding the date that the contest was conducted; c) employees of any AM, FM or TV
broadcast stations serving the San Diego area. Authentic identification clearly showing
the winner’s photo, name, residence address, and full birth date is required to claim any
prize.
7) Winner’s Responsibility. Each winner is solely responsible for payment of all prize
related fees or expenses not specifically mentioned, and for payment and reporting
of federal, state and local taxes and may be required to complete an IRS form W-
9. Each winner, by participation in the promotion and acceptance of the prize(s),
agrees to release KFMB Stations and their parent and subsidiary companies, their
officers, directors, employees, agents, shareholders, affiliates, suppliers, distributors,
and advertising agencies from all liability, claims, or actions of any kind whatsoever
for injuries, damages or losses to persons and property which may be sustained in
connection with the receipt, ownership, acceptance or use of the prize or promotion.
8) Promotional Use. KFMB Stations reserve the right to publicize and/or broadcast
participants/winners’ name, character likeness, voice, and any other matter incidental
thereof upon entering/winning for promotional purposes without further compensation to
the Promotion winner(s) or participants.
9) Responsibility of KFMB Stations. The KFMB Stations assume no responsibility for any
busy signal or inability of a contestant to call or log onto the station website to participate
in the contest, or for any malfunction of any equipment whatsoever, including telephone
equipment, computer or internet equipment. KFMB Stations are not responsible for lost,
damaged, late, misdirected, incomplete or illegible. Prize is forfeited if not claimed within
30 days. KFMB Stations are not responsible for failure to award the prize if winner is
disqualified or cannot be contacted via the email address or phone number provided.
10) Odds. The odds of winning depend on the total number of eligible entries received.
11) Rules Subject to Change. All rules are subject to change without notice. Any contest
can be suspended or canceled in the event that the integrity of the contest has been
jeopardized due to events beyond the KFMB Stations’ control. KFMB Stations will
interpret these rules and resolve and disputes or ambiguities; all decisions concerning
such disputes shall be final.
